The LEAP Study and the Paradigm Shift

The catalyst for this shift was the Learning Early About Peanut Allergy (LEAP) study. The study demonstrated that the early introduction of peanut products to infants at high risk for allergies significantly reduced the prevalence of clinical peanut allergy by age five.

The physiological mechanism is rooted in early immune tolerance. Exposing the immune system to these proteins early in life can help train it to recognize them as food rather than as threats.

Nutritional Value: Why Nuts Matter

Beyond allergy prevention, tree nuts and peanuts are sources of essential nutrients critical for early development:

1. Timing is Everything

For most infants with no history of severe eczema or existing food allergies, introduction can begin around six months of age, concurrent with the start of other solid foods.

2. The Form of Introduction

  • Nut Powders: Utilizing defatted peanut or almond powder without added sugars is highly precise. Mix the powder into fruit purees, oatmeal, or yogurt.
  • Dissolvable Snacks: Infants are sometimes given dissolvable peanut puffs. These can be softened with milk or water for very young infants.

3. The Introduction Protocol

  • Start Small: Begin with a minute quantity, approximately 1/4 to 1/2 teaspoon of the thinned butter or powder.
  • Timing of Day: Always introduce new foods in the morning or early afternoon, never right before a nap or bedtime. You need to be able to observe the child post-consumption.
  • Observation: Monitor for signs of an allergic reaction: hives, facial swelling, vomiting, or respiratory distress. If any of these occur, seek immediate medical attention.

4. Sustained Exposure

Tolerance is an active, ongoing immunological process. A single exposure is often insufficient. Current guidelines recommend offering the food regularly to maintain immune tolerance.
